Long-held belief in the disease model of addictive behavior has been that the maladaptive behaviors of those who use alcohol or other substances is a result of a chronic disease. These conditions are biological and can be exacerbated by environment factors. This conception renders the individual powerless over problematic behaviors and unable stay sober alone, just as terminally ill individuals are unable fight the disease themselves. Behavioral therapy, therefore, requires that the individual admit his or her addiction and abandon their former lifestyle. Then, they must seek out a supportive social circle to help them stay sober. These are the core features of twelve-step programs. They were first published in the book Alcoholics Anonymous (1939). Opponents have criticized these approaches on psychological and legal grounds.
