The long-held view of addiction as a disease has held that maladaptive patterns of alcohol consumption and substance use by addicted individuals is the result a lifelong illness. It is also exacerbated and caused by environmental conditions. This conception renders the individual powerless over problematic behaviors and unable stay sober alone, just as terminally ill individuals are unable fight the disease themselves. Behavioral treatment is therefore a process that requires people to confess their addictions, give up their previous lifestyle, and find support networks that will help them stay sober. These are the essential features of Twelve-step programs. These approaches have been met with a lot of criticism from those who disagree with the spiritual-religious orientation, both on legal and psychological grounds. It lacks scientific support for its efficacy, which is also a criticism. There is some survey-based evidence to suggest that there is a correlation in attendance and sobriety. There have been different results for other drugs. The twelve steps are less beneficial for people addicted to illegal substances than they are for people who are not. They also make it harder for people who use the psychologically and physiologically addictive opioids. These individuals require maintenance therapy, which is the gold standard in care.

Researchers have different ways of thinking about addictions. This influences the models they use. There are four main Behavioral Models for addiction: the Moral Model (or Disease Model), Socio-Cultural Model, Psycho-dynamic Model and Socio-Cultural Model. The Moral Model of Addiction holds that addiction is a result of moral weakness. Moral Model supporters see drug abuse as a choice for anyone, even those who are addicted. Addicts, on the other hand, are seen as people with bad character. According to the Disease Model, substance abuse can be described as a chronic, relapsing condition that causes brain damage. Research on the neurobiological causes of addiction has produced mixed results. The only treatment is abstinence. Socio-Cultural models attempt to explain why certain groups are more susceptible than others to substance abuse. It discusses the ways in which discrimination, poverty, lack of opportunity, or other problems that can be found in marginalized communities can lead to them becoming more vulnerable to addiction. Psycho-Dynamic models consider trauma and mental illness to be a precursor of addiction. Many rehabilitation centers offer treatment for "co-occurring" conditions, which is a combination of substance abuse disorder and a mental health diagnosis.
