Cracks or holes more than 1/4" wide are often a sign of a larger problem, and may require further investigation. Are roots growing underneath concrete causing cracks and lift? Is there a leaky line that is causing erosion below the slab which causes it to crack, sink or discolor? If this is the case, you may need to replace your driveway.
